Ann-Marie Dysktra

Hi, I’m Ann-Marie dykstra.

I’m a certified Speech-Language Pathologist (more commonly called Speech Therapist) with ASHA (American Speech-Language Hearing Association). I received my Masters in Speech-Language Pathology in 2010 from San Diego State University, while simultaneously being fortunate to intern at the VA Hospital. It was here I had truly found my calling working with the medically complex adult population. I then went on to complete my Clinical Fellowship at Mission Hospital, a large level II trauma center in southern California.  After 10 years working at various medical facilities across the United States, I decided to take the skills I acquired and open up my own mobile and teletherapy practice for adults serving Lake County, Florida. My hope is to bring much needed specialized treatment and healing for those in need of our services.

That’s right. We drive to your home. So you don’t have to go through the hassle of fighting traffic and waiting in a lobby room. Just relax and save your energy for our treatment session. In some cases we will provide services via video conferencing (teletherapy) if appropriate. This will be discussed within your initial call or evaluation.  Skilled evaluations are always done in person at your home while our wellness plans are done through teletherapy. 


Unlike most rehab companies that offer a variety of services, mine solely focus on Speech Therapy for adults. Our therapists have over 10 years of experience within complex medical conditions that impact communication, cognition and the ability to swallow.

We bill insurance

We accept Medicare Part B plans as well as most out of network insurance coverage. We do all the billing for you, so you only pay if you have a co-insurance/co-payment. Those payments can easily be set-up on your patient portal.

Certified Providers

We have therapists that are trained with NMES (often referred to as e-Stim or VitalStim) for Dysphagia (swallowing difficulties) as well as facial paralysis. For people with Parkinson’s Disease having problems with their voice, we have therapists that are LSVT Loud certified.

Effective Treatments

We team up with other professionals for optimal care as needed, particularly for those with head and neck cancer related swallowing problems. We choose treatments that are backed by evidenced based research utilizing the latest in technology when possible.

We determine together how many times to be seen based on your needs. If you are restricted by insurance, we have affordable Wellness Plan options that can offer much value as a supplement to an existing plan. 

Insurance & Costs


Health Insurance
  • Original Medicare plans under Part B Benefits

Cobalt Therapy is currently a Medicare B Provider and accepts all patients under original Medicare as well as those with insurance plans that cover out of network services. While we are not a provider for Medicare Advantage (or Medicare Replacement) plans, some will make exceptions if you can not find a Speech Therapist in their network for your area. 

We do all of the billing for you and you only need to pay if you have a co-payment/co-insurance portion. The only requirement for your care to be covered by Medicare is that you can not currently be in a home health episode receiving benefits under your Medicare Part A benefits. That would mean that you are already getting therapy services in your home along with nurse visits.

Other exceptions where Medicare will not cover services would be if you are already receiving Speech Therapy services (whether at home or outpatient). Medicare will not pay for duplication of services, so you can only have one plan of care active with a Speech Therapist at a time.

If you are not covered by insurance, or are in need of a service that insurance does not cover, we do offer private pay options, affordable Wellness Plans  and discounts if paid in advance. Please check out our  Teletherapy Wellness Plan on our Services page if this is you!

While a referral from your doctor is not required to get started with us, we will need a physician that is willing to sign off on the plan of care being covered by insurance when we fax it over to them.

Insurance can be very confusing, so call us and we will get your insurance coverage verified so you know exactly what to expect before you start your sessions.
